New Patients
What To Expect
New patients at the office can expect to fill out routine new patient paperwork, followed by a comprehensive series of radiographs and subsequent exams by both a dental hygienist and Dr. Euler. A hygienist will also perform a dental cleaning as indicated. It is our goal to address the patient’s chief complaint first and then identify any other existing or potential issues. Through this process, we can develop a plan of action that best fits the patient’s needs.
To help us serve you more efficiently at your first appointment, please print and complete our new patient form in advance and bring it with you.

It is the patient’s responsibility to verify that our office is in-network with their insurance provider. Coverage and benefits may vary depending on individual plans. Please contact your insurance company directly to confirm network status and benefit eligibility.
